Position Overview: Will be providing Client services by monitoring, assessing, and providing directed treatment common to complex injuries and illnesses through comprehensive assessments, including diagnosis, treatment, and outcome management for adult and pediatric patients. What to expect day-to-day as a Mobile Health Clinician: Each day, will be paired with a driver to patients' homes to provide care and monitoring. While at a patient's home, will engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare system Throughout the treatment process, maintain patient medical records and documentation consistent with state regulations, client standards, and policy through the HealthPoint and Athena EMR system Will be practicing under direction with synchronous telemedicine support Responsibilities: Act as a conduit for the medical treatment of the Health Plan Partnerships patient population Navigate conversations related to management of health, well-being, medication management, and goals of care Provide supervised, directed treatment of common medical diagnoses Participate in continuing education as required by the state where you reside and the Client training team Adhere to clinical and safety standards, protocols, and performance metrics Provide care with cultural sensitivity and compassion Lead your practice with a patient-centered approach Other related duties as deemed necessary Qualifications: Minimum of two years of experience as a Licensed Practical Nurse Active and current American Heart Association BLS Required Emergency Department, Urgent Care, Internal Medicine, Hospital – Critical Care, Family Practice or other relevant areas experience preferred Current unrestricted license NY State LPN Clinical work experience with pediatrics required Blood Draw/Phlebotomy Certification Required in addition to on the job experience drawing blood.
