Care provided by the LPN will be based on the RN's initial assessment of a patient and subsequent plan of nursing care.What you will do
- Assists the Registered Nurse with the collection, documentation, and communication of data for assigned patients. Provides for physical and comfort care.
- Observes and reports any relevant information and/or changes in the patients' condition to the Registered Nurse. Documents and communicates patient responses and the effectiveness of nursing interventions.
- Routinely utilizes Evidence Based Practice literature, policy and procedure manuals and other available reference materials to ensure consistency and accuracy in the delivery of care.
- Reinforces patient teaching plans as written in the Plan of Care.
- Completes all required educational programs annually. Demonstrates understanding of safety and infection control practices, emergency procedures, fire drills, evacuation procedures, and internal/external disaster plans and procedures
- Evaluates the activities/services of ancillary personnel providing nursing care in collaboration with the Registered Nurse.
- Participates actively in the Performance Improvement process to evaluate the quality of nursing care delivered to patients.
- Understands and demonstrates compliance with all hospital and departmental policies and procedures.
- Participates in unit-based and organization-wide activities, committees and community activities as appropriate.
- Summary of Skills
- Provides wound care including dressing changes or application of wound vac under the supervision and guidance of the registered nurse.
- Collects blood, urine and POC glucose specimens, appropriately
- Consistently identifies all patients and specimens per hospital and laboratory policies.
- Assures phlebotomy is being performed on the correct patient by using two patient identifiers.
- Maintains low blood culture contamination rate by using correct blood culture procedures.
- Recognizes problems unique to different age groups and adjusts treatment procedures accordingly.
- Administers medication in accordance with hospital policy.
- Insertion and maintenance of intravenous access.
- Midline and central line dressing changes.
Education Qualifications
- Graduate from an accredited School of LPN/LVN required
Experience Qualifications
- Acute or Sub acute experience preferred
Licenses and Certifications
- LPN - Licensed Practical Nurse - State Licensure Current unencumbered licensure as an LPN in the state of New Jersey upon hire required and
- BCLS - Basic Life Support upon hire required and
