Job Description:
We are currently seeking *LPNs *for a Long term care facility in the Manhattan Area.*Available Shifts*:* 7am-3pm* 3pm-11pm* 11pm-7am*Overview: *Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. The duties require the use of tact and judgment in the care and treatment of patients. May assist in the supervision of non-professional subordinates.  Is cognizant of philosophy, standards, objectives and policies of the Nursing Department.  Performs related duties as required.*Job Duties:*Provides direct nursing care to assigned residents with complex needs as related to:* Hygiene and physical comfort* Medical treatment* Nutritional needs* Ambulation and exercise* Elimination Maintenance of residents' safety* Maintenance of residents' Safety* Directs the care of the residents performed by Certified Nursing AssistantsAdministers medication to assigned residents with exception of the following:* Blood and blood products* Hanging the first IV solution containing medications* Administering IV (push) medication* Administering IV medications via central Lines and Maintenance.*Responsibilities: *Demonstrates proficient knowledge in the treatment of patients requiring nursing care. Uses available diagnostic results and other information to make an accurate evaluation. Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process. Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted. Is considerate of residents' privacy during the evaluation process. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals Contributes to the plan of care making necessary changes as the condition of the patient indicates. Bases care decisions on resident needs and care priorities. Monitors patient outcomes. Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement. Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner.*Qualifications:** Valid NYS Licensed Practical Nurse (LPN) license* Completion of LPN program* Demonstrates competence, and experience in nursing skill within the scope of practice.* IV Certification* CPR/BLS through American Heart Associated* Vaccinated for COVID-19* Minimum 6 months of long term care experience* Ambulatory Care experience preferredJob Types: Full-time, Part-time, ContractPay: $37.00 - $39.00 per hourExpected hours: 40 per weekBenefits:* Health insurance* Paid sick time* Paid time off* Paid trainingMedical Specialty:* GeriatricsPhysical Setting:* Long term care* Nursing home* Rehabilitation centerEducation:* Associate (Preferred)Experience:* Long term care: 1 year (Preferred)License/Certification:* CPR/BLS Certification (Preferred)Work Location: In person%{{advertiserId}}% %%{{category}}%%
