Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents.Work Environment:Work is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided.Responsibilities:•Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.•Monitoring and Assessment: Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.•Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.•Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.•Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.•Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.•Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.Qualifications:•Licensure: Valid LPN license in Nebraska.•Education: Graduation from an accredited LPN program.•Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.•Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.•Communications: Excellent verbal and written communication skills.
