- Current and valid NYS LPN License
- Minimum of one year acute or long-term care experience required
- Must be 18 years old
- Must be able to lift a minimum of 50 pounds
Responsibilities for a LPN: Licensed Practical Nurse:
- Provide direct care and treatment to individuals in assigned program(s) through a variety of nursing procedures and medication administration.
- Respond and assist in medical emergencies and take appropriate action with guidance from RN.
- Observe and monitor individuals with specific health problems and those at risk and report pertinent findings to RN.
- Document all current health related information in nurses' notes and/or medical record.
- Check nursing equipment as indicated.
- Assist RN in annual assessment of consumers in day programs.
- Keep medical files current and complete.
- Attends nursing/program meetings as scheduled.
- Communicates with other service providers, program staff, administration, etc., re: changing medical issues.
