Schedule: Full-time and part-time shifts available
Pay: $30.00 - $35.00 per hour
Expected hours: 20 – 40 per week
Responsibilities:
- Providing care according to the patient's plan of care, assistance with daily tasks, which include wound care, injections, assessments, vitals, glucose checks, blood drawing, treatment plans, record keeping, overseeing CNA/PCA, med passes, etc.
- Conduct ongoing assessments.
- Participate in health/therapeutic counseling, teaching, and emotional support to the client and family/caregivers.
- Assist RN with client discharge or transfer.
- Recognize and institute when emergency measures are needed in a rapid, appropriate manner.
- Participate in the coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e. PT, OT, ST) to the DON or RN supervisor.
Requirements:
- High school diploma or GED
- Current Ohio LPN License in good standing.
- BLS Certification.
- Licensed driver with insured and reliable automobile.
- Self-directing with the ability to work with little direct supervision.
- Empathy for the needs of the client.
- Ability to read, write, speak, and understand English and communicate effectively.
- Treats clients, staff, and the public with courtesy, respect and presents a positive public image.
- Works as a team member.
- Ensures confidentiality and security of the client's medical information.
- Must be able to stand walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift to 50 pounds.
