*Job Overview*We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience working with individuals with developmental disabilities and possess strong clinical skills. This role requires a commitment to providing high-quality patient care in various settings, including inpatient facilities and clinics. As an LPN, you will play a vital role in supporting patients' health and well-being through effective communication, medication administration, and collaboration with healthcare professionals.*Responsibilities** Provide direct patient care under the supervision of registered nurses and physicians.* Administer medications and monitor patients for adverse reactions.* Perform IV insertions and assist with other clinical procedures as needed.* Participate in discharge planning to ensure smooth transitions for patients leaving care.* Document patient information accurately using electronic health record systems such as Epic.* Assist in the assessment of patients' needs and develop appropriate care plans.* Support patients with daily living activities while promoting independence.* Collaborate with multidisciplinary teams to provide comprehensive care.* Maintain a safe and clean environment for patients and staff.*Requirements** Valid Licensed Practical Nurse (LPN) license in the state of practice.* Experience working with individuals with developmental disabilities is preferred.* Proficiency in medication administration and basic nursing procedures.* Familiarity with electronic health records, particularly Epic, is a plus.* Ability to perform IV insertions and other clinical tasks as required.* Strong communication skills to effectively interact with patients, families, and healthcare teams.* Knowledge of discharge planning processes is beneficial.* Willingness to lift patients or assist them in mobility as needed.* Experience or training in sonography or urgent care settings is an advantage.Join our team of dedicated healthcare professionals committed to making a difference in the lives of our patients.
