SUMMARY OF JOB:Working under the direction of supervisor and/or Nurse Manager, directs and takes responsibility for total care of residents on a team. Responsible for implementing interventions designed to achieve the goals and objectives of the resident plan of care.
CONNECTING TO MISSION:
All work is performed within a resident centered frame work in collaboration with other healthcare professionals, residents and their families. Functions as a role model for staff. Recognizes the value of each person as an individual. Supports and Promotes Resident/Patient strengths and abilities, capacity for social contribution, unique values, preferences, living habits and autonomy of choice. Embraces a vision of aging that is meaningful and relationship based.
RECRUITMENT REQUIREMENTS:
Current LPN license
Previous experience preferred
Demonstrates position communication skills
Ability to relate to staff in a professional manner
Must be flexible with scheduling
Computer experience preferred
Demonstrates sensitivity to residents emotional, social, mental health and spiritual needs
Previous experience in a rehab setting preferred.
Must be able to interact well with physicians and other health care professionals
Able to exercise sound professional judgment
Able to respond to emergency
LICENSE AND REGULATORY REQUIREMENTS
