Job Description:

The LPN/LVN - Hospice is responsible for providing care,comfort and support to terminally ill patients. The LPN/LVN - Hospice assumesthe responsibility and accountability for the delivery of hospice patient carein various settings. The LPN/LVNdemonstrates the ability to make clinical judgments in an effective andefficient manner with supervision. The LPN/LVN will also demonstrate criticalthinking and performance ability in the coordination of patient care. Essential Duties and Responsibilities: Documents the patient's plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventions.Identifies and prioritizes actions based on patient care requirementsPerforms interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standardProvides a safe, comfortable and therapeutic end-of-lifepatient environmentProvides patient and/or family education, guidance andsupportDocuments the patient's response to nursing and otherinterventionsUses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing prioritiesMonitors safety and effectiveness of the environment andequipmentAdvises grieving family members on coping techniques andcommunity resources • Performs other duties as assigned/necessary Minimum Requirements: Current Licensed Practical Nurse/Licensed Vocational Nursefor the state in which the nurse practicesComplies with all relevant professional standards ofpracticeParticipation and completion of Amergis' Competency programwhen applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or stateregulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable tojob siteUnderstand patient confidentiality and HIPAArequirementsAbility to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is requiredComputer proficiency requiredMust be at least 18 years of age Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecksHealth, dental, vision, and life insurance401(k) savings planAwards and recognition programs *Benefit eligibility is dependent on employment status.
