*Job description**Join us for a people-first culture and choose APN Healthcare Solutions for a staffing experience that prioritizes your success above all else.*APN Healthcare Solutions is urgently hiring multiple *Licensed Vocation Nurses *for our client in Crescent City, CA for a 13 week contract with a high possibility of extension.*Title*: LVN/LPN - *Licensed Practical Nurse**Location*: Watertown, NY*Shift /Hours/day*: Evenings / 40 hrs / 3p-11:30p*Duration*:13 weeks*License Requirement*: New York LVN/LPN*Certification Requirement*: BLS_*Required Experience : 1-3 years Exp (SNF experience, not just LTC)*_*Job Description:*The *Samaritan Keep Home*, a 272-bed long-term care facility connected to the *Samaritan Medical Center.* This position is ideal for individuals with *1-3 years of experience* in a skilled nursing facility (SNF), and we are especially interested in candidates with experience in providing care for residents with conditions such as dementia, chronic diseases, and diabetes.*Key Responsibilities:** Provide direct patient care to residents, including medication administration, wound care, and monitoring of vital signs.* Develop, implement, and evaluate individualized care plans for residents, ensuring that their health needs are met.* Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate care and ensure the best outcomes for each resident.* Perform assessments and document all patient care in the *Point Click* Electronic Medical Records (EMR) system.* Educate residents and their families about care plans, health conditions, and treatments.*Required Qualifications:** *1-3 years of experience* as a Registered Nurse, preferably in a Skilled Nursing Facility (SNF), though candidates with experience in Long-Term Care (LTC) may also be considered.* *New York State Registered Nurse License* (active and in good standing).* *Basic Life Support (BLS)* certification from the American Heart Association (AHA) required.Job Types: Contract, Travel nursingPay: $1,850.00 - $1,940.00 per weekPhysical Setting:* Long term care* Nursing homeSupplemental Pay:* Overtime payPeople with a criminal record are encouraged to applyExperience:* SNF/LTC: 1 year (Required)License/Certification:* New York license (Required)* BLS Certification (Required)Work Location: On the road
