The Long Term Care LPN assumes responsibility andaccountability for the delivery of care for patients in a Long TermCare/Assisted Living Facility. The nurse demonstrates ability to make clinicaljudgments in an effective and efficient manner with supervision, as well ascritical thinking and performance ability in the coordination of patient care.Additionally, this individual performs according to nursing standards and isaccountable in managing patient care and assisting others in the management ofpatient care.Essential Duties and Responsibilities:Utilizes the nursing process to assess, plan, implement andevaluate patient care in a Long Term Care/Assisted Living FacilityPerforms focused interview to identify specific needsAssess signs and symptoms indicating physiologic andpsychological changes in the patient's conditionCollects, analyzes, and interprets data and information fromhealth care members and documents actual and/or potential nursing diagnosesDocuments the patient's plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventionsIdentifies and prioritizes actions based upon patient carerequirementsMonitors consistency between patient plan of care andactions of medical staffPerforms interventions according to identified priorities,plan of care, and the facility policies and patient outcome standardProvides a safe, comfortable and therapeutic patientenvironmentInitiates and evaluates patient and/or family educationRevises the plan of care according to evaluation, changes inmedical plan of care, and effective/ineffective nursing interventionsDocument's patient's response to nursing and otherinterventionsUses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing prioritiesMonitors safety and effectiveness of the environment andequipmentParticipates in activities to facilitate good interpersonalcommunications with the chosen facilitySupports professional practice by observing the following:Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2)Standards of Nursing Practice; 3) Standards of Patient Care; 4) JointCommission, state and local laws; 5) Patient's Bill of Rights; and 6) AmergisHealthcare, nursing and client hospital policiesPerforms other duties as assigned/necessaryMinimum Requirements:Current Licensed Practical Nurse License for the state inwhich the nurse practicesAbility to effectively elicit/provide information to andfrom appropriate individuals via strong spoken, written, listening, andnonverbal communication skillsCurrent Health Certificate (if applicable)Current PPD or Chest X-RayOne year prior professional nursing experience preferredCurrent BLS cardSuccessful completion of screening tests for the clinicalspecialty area(s) in which the LPN practices with a score of 80% or greaterSuccessful completion of Background Screening ProcessBenefitsAt Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits:Competitive pay & weekly paychecksHealth, dental, vision, and life insurance401(k) savings planAwards and recognition programs*Benefit eligibility is dependent on employment status.About AmergisAmergis, formerly known as Maxim Healthcare Staffing, has served our clients and communities by connecting people to the work that matters since 1988.
