LPN Nurse
Job Description
The primary functions of an LPN involve assisting with the care of all patients, regardless of age, during his/her shift. The LPN is responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. The LPN effectively provides Person Centered care and services.
Responsibilities include:
- Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate, and reporting changes in the patient's/residents conditions
- Practicing ethically and professionally within their scope; advocating for patients
- Maintaining clinical competence specific to their care area, including organization-required education within the stated timeframe
- Coordinating care with the interdisciplinary team
- Documenting appropriately and thoroughly in EMR/Point Click Care
- Keeping other nursing personnel and the responsible RN informed regarding changes in the condition of patients/residents and/or other pertinent observations
- Participating in unit councils and staff meetings.

Work Site
This is a fully on-site position in Montour Falls, New York.
Work Environment
2 full-time LPN positions available. Every other weekend and every other holiday required. No overtime will be allowed during the assignment.
