At NY Health, all of our patients are family. In this role, you will:
  • Provide ongoing support relative to clinical data extraction, collection, and submission
  • Assist in the coordination of outreach efforts to request, collect, and secure clinical documentation from Primary Care Providers (PCP) and/or Specialist(s)
  • Review assigned medical records to abstract specified and required clinical data elements related to gap closure
  • Assist with training of staff at PCP practices and provide feedback on processes and documentation
  • Attend monthly meetings with insurance companies, navigate portals and maintain appropriate documentation to track performance
  • Other duties and responsibilities, as assigned
We require:
  • Current NYS LPN license required.
  • Minimum of three (3) years of experience in the healthcare industry, in HEDIS and/or quality improvement.
  • Experience in a managed care health services environment preferred
  • Demonstrated knowledge of data collection, medical record review, and data submission.
  • Ability to effectively prioritize work, multi-task, and achieve goals on time; work independently and self-motivate
  • Proficiency in the use of Electronic Medical Records and computer applications (Google Suite, Microsoft)
  • Excellent oral and written communication skills
Hours: Monday-Friday Salary up to $35 per hour based on experience and education.
