Job Description:

The Licensed Practical Nurse provides nursing care to patients under the supervision of RNs or licensed providers in accordance with physician orders and established guidelines. The LPN provides and maintains a clean, safe, and quiet environment of support and comfort to patients from birth through the lifecycle.

Major Responsibilities

* Assures Quality Patient Care

* Participates in performance improvement activities as assigned.

* Participates in continuous quality improvement activities within the hospital and/or nursing unit as requested.

* Provides Direct Patient Care

* Follows existing protocols.

* Completes designated components of the Admission Assessment.

* Assists in admission, by preparing patients room. Orients patient and/or significant other to environment.

* Observes patients while in patient rooms and immediately reports any adverse signs, symptoms or unusual occurrences to the physician and charge nurse.

* Provides support to the charge/staff nurse or CNA who requires assistance with bathing, dressing, turning, toileting, or ambulating patients who require two-person assistance.

* Changes bed linens on unoccupied and occupied beds.

* Collects and appropriately labels specimens including wound, stool, sputum and urine, strains urine as appropriate. Assists with menu selection, sets up trays, assists with feeding as needed and collects trays.

* Checks for changes in diet orders, monitors trays for correctness, records patient's dietary intake. Measures, calculates, and records the patient's intake and output.

* Passes nourishment, water, juices, or ice.

* Monitors patients in restraints and documents per policy.

* Transports patients from unit for discharge or procedures. Adheres to principles of infection in performing direct patient care.

* Performs other assigned duties as appropriate for training/competency level.

* Administers medications observing the five medication rights and per FLOYD policy, PO, IV, IM, Sub-q, enteral and rectally. Documents patient response to medication as indicated.

* Administers blood or blood products according to the FLOYD policy. Inserts IV line and cares for IV site according to FLOYD policy.

* Counts, wastes, and picks up narcotics from pharmacy per FLOYD policy. Records and receives report per Voice Care.

* Inserts and cares for nasogastric tube per FLOYD policy. Inserts and cares for foley catheter per FLOYD policy.

* Changes sterile and non-sterile dressing. Checks physicians orders, takes verbal orders.

* Checks MAR and makes necessary adjustments.

* Documents patient records legibly and as required per FLOYD policy. Performs Bedside Blood Glucose.

* Obtains vital signs. Uses the Doppler

* Uses the pulse oximeter. Uses scales.

* Removes staples and sutures. Uses the I.V infusion pump.

* Completes medical record reviews as assigned. Completes other duties as assigned.

* Performs tracheotomy care. Performs suction.

* Uses CPM.

* Uses pulsatile hose.

* Applies an external catheter.

* Cares for a patient with a Peg tube.

* Changes dressing on a PICC, central line per FLOYD policy. Uses the bladder scanner.

* Participates in the bowel and bladder protocol. Competent to apply telemetry pads.

* Uses the FIM

* Completes other duties as assigned.

* Provides Indirect Patient Care

* Keeps patient rooms neat and orderly, making beds occupied and unoccupied. Monitors room availability.

* Disposes of biohazardous trash.

* Separates and distributes linens to patient rooms. Replenishes supplies in patient rooms.

* Transports specimens if STAT, tube system own or specimen cannot be sent via tube. Notices supply and maintenance problems and reports to the appropriate person.

* Obtains and returns supplies as requested.

* Performs safety/control checks on unit equipment as per unit standards. Takes necessary safety precautions and reports unsafe situations.

* Completes other duties as assigned.

* Reflects Appropriate Nursing Process Under the Direction on the Charge Nurse and/or Staff RN

* Provides input into the development of the patient's interdisciplinary plan of care. Documents on the medical record accurately and legibly.

* Reports pertinent data to appropriate health care team members. Implements patient care as directed by the Charge or Staff Nurse.

* Consistently Participates in Activities which Promote Customer Satisfaction

* Assists the team in meeting or exceeding satisfaction expectations as measured in patient experience surveys.

* Continuing Education

* Completes competency checklist within first 90 days. Completes unit recommendations within 1 year of hire date.

* Obtains at least 16 hours of continuing education, minimum 10% unit specific Completes unit and departmental annual skills checklists.

* Successful completion of Pharmacology exam

Education

Graduate of an accredited school of Practical Nurses

Certification / License

Valid, active State of Georgia LPN license for GA locations and State of Alabama LPN license for AL locations BLS certification

Work Experience

* 1 year of equivalent work experience required
