NOW HIRING: LPN Licensed Practical Nurse
Fishkill Home Care has been servicing the residents of Dutchess County since 1982. We offer a team of experienced and dedicated Nurse Case Managers, who work together with patients, families, doctors and health care facilities to provide a comprehensive home care program for each client. The primary purpose of this job is to provide skilled nursing care and prescribe treatments in patients homes.
**SEEKING FULL TIME PNs in Dutchess County & Putnam County, NY**
The LPN has the choice to choose their service area within Dutchess and Putnam Counties.
Responsibilities and Duties
- Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of home bound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care.
- Uses health assessment data to determine nursing diagnosis.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Regularly re-evaluates patient nursing needs.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Manage, evaluate and coordinate care plan implementation with all related disciplines.
Specific Requirements:
- NYS Licensed Practical Nurse (LPN)
- One year of experience required
- Experience with Acute Care preferred
- Must possess the ability to deal tactfully with personnel, clients, family members, government agencies/personnel and the general public
- Must have a valid Drivers License
- Must have patience, enthusiasm and a cheerful disposition
