Why does the job exist?Responsible for utilizing the nursing process to provide direct resident care to an assigned group of residents during a shift. Responsible for prioritizing the delivery of direct nursing care using time and resources efficiently, integrating a standard based framework model. Provide clinical supervision for non-professional staff. How does it get done?Maintain accurate, detailed reports and records. Administer medications to patients and monitor patients for reactions or side effects. Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patients' conditions. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Who are the customers?Residents at a long-term care facility. Ideal CandidateWill have:The ability to read, write, speak, and understand the English language. The ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities. The ability to function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. Minimum QualificationLicensure as LPN or LVN, and two (2) years of experience as it pertains to the essential duties and responsibilities of the classification. Any combination of education from an accredited college or university in a related field andor direct experience in this occupation totaling two (2) years may substitute for the required experience. Employment RequirementsThis position is designated as safety sensitive. Employment is subject to pre-employment and random testing in accordance with all terms and conditions of federal and state law, rules and regulations in relation to alcohol and/or drug testing. Employment is subject to nationwide criminal background check and is conditional pending results. Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Certification from the State of New Mexico or must pass and receive a Defensive Driving Course Certification as a condition of continued employment. Licensure to practice as a Licensed Practical Nurse in New Mexico. Successful completion of the Health Insurance Portability and Accountability (HIPPA) training. Working ConditionsWorks in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses¿ stations, resident rooms, etc.).Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents¿ needs. Is subject to exposure to infectious waste, diseases, conditions, etc.Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.State of New Mexico employees may be impacted by the Public Health Order dated December 19, 2022, which requires facilities licensed or certified by the Centers for Medicare and Medicaid Services ( CMS ), including all hospital types, long-term care facilities, nursing homes, hospice facilities and rehabilitation facilities, as well as assisted living facilities and adult day care settings, to adhere to all COVID-related requirements prescribed by CMS, including, but not limited to masking and staff vaccination.
