Younker 8 is a 23 bed Medical Surgical unit that provides an enriching learning experience while caring for patients with a variety of diagnoses specializing in pulmonary conditions. Younker 8 offers opportunities to advance critical thinking skills and nursing autonomy within our multidisciplinary team. Our nursing team has a voice to advocate for change in the nursing profession. As an LPN in this care model, you will provide care for a specified group of patients, utilizing all the skills you have gained in obtaining your license. With the support of an RN and other team members of the care group, you will perform tasks or skills within your scope of practice, including but not limited to:
- Provide safe and reliable patient centered care with support and supervision of an RN.
- Collaborate with multidisciplinary team members.
- Manage care with ongoing assessments and vitals.
- Administer medications as ordered by the provider.
- Provide evidenced-based patient education starting at admission.
- Document daily progress toward care plan goals.
- Complete bedside procedures (e.g., foley, NG, dressing changes, drains, etc.).
- Provide discharge education and review instructions with patient and/or caregivers.
- Document in the electronic medical record

Responsibilities:
Clinical Excellence & Patient Care
- Possesses the knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness
- Participates in patient/family educational activities and uses appropriate resources to implement an individualized teaching and discharge plan with patients and their families
- Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for patients
- Reinforces the care provided by other team members
- Provides direction and supervision of unlicensed assistive personnel (e.g., Patient Care Technician), demonstrates clinical judgment appropriate for the states LPN scope of practice, uses problem solving techniques and time-management principles, and delegates appropriately
- Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals
- Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and health team members and to ensure the fulfilment of requirements for legal documentation and reimbursement
- Maintains a strong commitment to the multidisciplinary health care team
- Contributes to the assessment of the physical, psychological, sociocultural, and spiritual dimensions of patients and their families, as well as their educational and discharge needs
- Collaborates with other rehabilitation team members and contributes to the dynamic interdisciplinary plan of care
- Implements the interdisciplinary plan of care within the scope of LPN/LVN practice and provides nursing care and education directly or through ancillary personnel, as needed, to maintain and restore function and prevent complications and further loss
- Contributes to the evaluation of the nursing care and to the modification of the plan as needed to achieve measurable goals and objectives
- Contributes to the interdisciplinary team process at team conferences and other team meetings and offers input into team decision making within their scope of practice.
- Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient.
- Collaborates with the staff nurse and other team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations
- Communicates effectively and interacts in a manner that facilitates the group process and team building.
- Advocates and supports the rights of patients and families with care decisions.
- Practice reflects knowledge of federal/state regulatory and accrediting agencies (e.g., Department of Inspection and Appeals (DIA))
