Role and Responsibilities This position coordinates and ensures compliance to the Face-to-Face client visit with their primary care physician requirement. This role involves coordination and follow-up with clinical staff and physicians. Essential Functions
- Process Workflow in HCHB to produce Face-to-Face documents for physician signature, in accordance with the regulations.
- Run reports and monitor compliance of returned forms and review for adequacy of documentation to meet the intent of federal regulations.
- Coordinate with physicians/physician's office staff and HCR clinical staff, as needed, to procure forms not completed.
- Review returned documentation to ensure clinical components of the requirements are met.
- Process documents through HCHB to allow billing to occur.
- Other duties as assigned.

- Associate's degree required.
Qualifications and Requirements
- Current, valid LPN license from NYS Department of Education required.
- This role will require critical thinking skills, strong organizational skills, and excellent communication skills.
- There is also a need for clinical oversight/interpretation when reviewing the documentation.
- Must possess basic computer skills - Email, Internet, Microsoft Office.
Work Environment The Face-to-Face Coordinator LPN is primarily in an office setting and may be exposed to outdoor conditions. The working conditions are classified as sedentary work:
- Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
