PRIMARY RESPONSIBILITIES:Coordinate the required testing and follow up for potential lung transplant candidates, lung transplant candidates (waitlist management) and lung transplant recipients. Maintain adherence to regulatory requirements of the United Network Organ Sharing (UNOS) bylaws,Centers for Medicare & Medicaid Services (CMS) and Joint Commission (JC) regulatory bodies. The transplant coordinator will share on-call responsibilities for the lung tranplant program, which includes collaborating with Gift of Life, medical and surgical teams on the inpatient and outpatient services, all members of the multidisciplinary team and consulting specialists as indicated. The transplant coordinator will also support the out patient transplant clinics, round on in patients, coordinate the follow up care of transplant patients in all phases of transplant. ESSENTIAL FUNCTIONS:Please list the fundamental job duties of the position (approx 3-5).¿ Coordinate evaluation of potential heart transplant recipients¿ Participate in outpatient clinic hours, inpatient rounding and the 24/7/365 on-call schedule rotation¿ Activate and provide clinical follow-up and maintain program protocols for patients on the transplant waitlist¿ Coordinate the admission process for admitting patients into the hospital for transplant and waitlist maintenance¿ Coordinate the urgent and emergent evaluation process for transplant candidates¿ Maintain regulatory requirements for CMS and UNOS .¿ Coordinate ongoing care, progress and treatment of lung transplant recipients following transplantation¿ Provide post-operative teaching and continuous education to post transplant recipients¿ Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.
Qualifications - Bachelor's of Science in Nursing Required
- Previous critical care and/or CICU/ CV surgical ICU experience preferred
