Job Description:

Wellness Program / HCC manager under the direct supervision of the Director will provide oversight and coordination of care for senior members within the Wellness Program. Manager oversee & coordinate outreach, give direction to Wellness staff of activities required to schedule annual wellness exams for designated seniors. Manager will report to senior leadership productivity data regularly, will track statistical metrics and develop a reporting platform. The Wellness Program/ HCC manager will oversee Wellness teams in North Hollywood and West Covina.

Essential Duties and Responsibilities include the following:

  • Manage and oversee the outreach and scheduling of annual wellness visits for IPA and staff model Medicare Advantage members to achieve target productivity goals
  • Establish relationships with select IPA provider offices to coordinate scheduling of IPA annual wellness visits performed by employed wellness providers
  • Recruit, onboard, and train wellness staff
  • Conduct regular team huddles to monitor progress against performance targets
  • Monitor and generate productivity reports, wellness visit metrics, wellness visit completion rates, screening test completion rates, and other risk adjustment reports for all staff model offices
  • Collaborate with internal stakeholders in the development of marketing collateral and other patient-facing material for the wellness program
  • Oversee coordination of rideshares for wellness visits
  • Create and update workflows, onboarding/training, and policies and procedures
  • Perform medical record audits to ensure that all pertinent ICD-10 codes are captured and that documentation supports such diagnosis codes
  • Present risk adjustment performance reports at each site's monthly provider meeting
  • Assist in developing outreach scripts.
  • All other duties as directed by management.

Education and experiene requirements:

  • 3+ years of experience in overseeing Medicare risk adjustment programs in a medical group/IPA setting
  • Experience using EMR
  • Experience using Microsoft Office, Excel, Word, Power Point and Adobe
  • High School Diploma required Associates degree or Bachelor's Degree preferred.
  • Knowledge of HMO, Knowledge of CPT, HCPCS, Hospital Revenue, and ICD-9 coding.
  • Strong knowledge of Microsoft Windows environment such as Word and Excel.
  • Self-starter, willing to take on multiple tasks.
  • Strong analytical skills.
  • Must be able to review and interpret health plan benefits and provider contracts.
  • Ability to communicate effectively orally and written typing 35 words per minute.
  • Strong organizational skills with emphasis on prioritizing and attention to details.

The pay range for this position at commencement of employment is expected to be $80,000 /yr to $90,000/yr however, base pay offered may vary depending on multiple individualized factors, including market location, job related knowledge, licensure, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.
