Job Description:
  • Select, train, and manage designated staff, fostering a work environment that encourages teamwork, promotes personal growth and learning, couples empowerment with accountability, learns from failure and celebrates success.
  • Manage all case management/health coordination activities ontheir designated island(s) ensuring all assessments, health action plans/care plans are completed and implemented according to Med-QUEST, Medicare, or commercial line of business requirements.
  • Administer an effective program through multi-disciplinary teams with the goal of improving health care outcomes and member satisfaction.
  • Identify opportunities and develop programs to improve the operational efficiency and cost-effectiveness of case management/health coordination processes and programs.
  • Oversee the performance of their team on their assigned island to ensure that appropriate services are being provided. The manager will maintain a positive working relationship and ensure that deliverables are being met in accordance with contract requirements.
  • Establish and maintain cooperative and productive professional relationships with health coordinators and case managers at various facilities and community organizations.
  • Act as a liaison and mediator between participants, families, and service providers addressing any concerns that may arise.
  • Attend and participate in meetings/workgroups/committees for community organizations and serve as resource/liaison between HMSA and the community.
  • Performs all other miscellaneous responsibilities and duties as assigned or directed.

  • Bachelor's degree and four years of related work experience; or equivalent combination of education and related work experience.
  • One year of supervisory, management, or leadership experience.
  • A current Hawaii license of LCSW, LSW, RN, LMHC, or other licensed clinician.
  • Effective verbal and written communication skills.
  • Working knowledge of the operation of managed care programs.
  • Basic knowledge of Microsoft Office applications. Including but not limited to Word, Excel, and Outlook.
  • LCSW, LSW, LMHC, or RN license valid in the State of Hawaii.
    • (upon hire, proof of licensure to be provided by employee to Human Resources as well as a signed Clinical Affirmation Statement for RN/LSW form if applicable).
  • Reliable transportation to perform in person (Face to Face) meetings or in alternative settings.
