Summary of Position

  • Responsible for the overall management and education of care team members providing professional nursing care to
patients. Proactively support efforts to ensure that standards for quality of care, patient satisfaction, and patient safety aremaintained, as established by AdvantageCare Physicians including, best practices, regulatory requirements, and clinicalworkflows. Serve as a leader, role model and liaison for the care team, ensuring quality patient-centric care at the medicaloffices.
  • Work closely with practice leadership to ensure practice goals and metrics are achieved.
  • Provide educational support for all care team activities.
  • May manage care team staff for up to two medical offices.
Principal Accountabilities
  • Manage the daily operations of the medical office care teams including, ensuring that standards for quality of care,
patient satisfaction, and patient safety are maintained, as established by AdvantageCare Physicians (ACPNY).
  • Assess the practice needs to identify gaps in care teams and assist with recruiting and retention efforts as
appropriate. Screen and select applicants and provide general orientation to newly hired team members. Providework direction, assign schedules, and conduct on-the-job training. Monitor and evaluate care team performance, andrecommend personnel actions, such as promotions, demotions, transfers, and disciplinary actions.
  • Assist in the interpretation of and compliance with Human Resources, ACPNY and departmental policies and
procedures.
  • Ensure employees understand and comply with local, state, federal and regulatory standards, as they apply to
respective job functions.
  • As a member of the interdisciplinary healthcare team, collaborates with providers and others on the clinical care
team to formulate and implement comprehensive patient centered plans of care to achieve expected outcomes.
  • Interface with physicians and all staff members as it relates to care team process.
  • Ensure compliance with all infection control policies, procedures and OSHA (Occupational, Safety and Health Act)
standards. Participate in program evaluation and quality improvement activities; actively seek input to supportpatient care and outcomes. Ensure all HEDIS (Healthcare Effectiveness Data and Information Set), HCC (HierarchicalCondition Categories) and PCMH (Patient-Centered Medical Home) measures are completed in the required timeframe.
  • Collaborate with the Quality team to identify areas in need of improvement as well as sharing of Best Practices.
Report all metrics related to patient outcomes and work collaboratively with the Director, Nursing and the QualityDepartment.
  • Maintain all regulatory certifications including but not limited to AAAASF (American Association for Accreditation of
Ambulatory Surgical Facilities), AAAHC (Accreditation Association for Ambulatory Health Care), Joint Commission, andCMS (Centers for Medicare & Medicaid Services) Deemed Status.
  • Continually evaluate clinical workflows for improvement opportunities, engaging and informing staff as appropriate.
  • Provide oversight to ensure daily huddles are occurring and in-basket messages are responded to in a timely manner,
address delinquencies with appropriate staff.
  • Collaborate with Case Management team members on complex cases as needed.
  • Ensure nursing practices of the care team as well as own practices are in accordance with NYS Department of
Education nursing standards.
  • Responsible for clinical in-service training, ongoing education within the practice for all clinical staff as well as own
professional development through periodic attendance at ongoing educational events.
  • Provide staff education by teaching and supporting staff in the care team process. Promote and coordinate care
team educational programs in the individual office setting.
  • Ensure required competency of procedural staff, with associated proof/documentation.
  • Evaluate staffing needs and make changes to nursing staff schedules to ensure proper coverage for all areas.
  • Collaborate with the Practice Administrator to create and maintain schedules, oversee facility maintenance and
report any environmental hazards for immediate resolution.
  • Review Facility Codes/Disaster plans. Ensure all staff is oriented to fire/emergency procedures. Conduct/facilitate and
document/evaluate drills (fire, disaster, code blue). Ensure that evacuation maps, PASS/RACE signs are located andmaintained as appropriate. Ensure fire extinguishers are checked annually by outside vendor and monthly by facility.
  • Collaborate on the development of new programs within the organization.
  • Support implementation of programs and procedures developed at an organizational level.
  • Collect data and adjust processes to accommodate program requirements.
  • Participate in ongoing nursing leadership meetings; bring relevant information back to clinical team for
implementation.
  • Order medical supplies for the medical offices and reconcile purchase orders. Continually review products and their
value to ensure the ordering is appropriate to the facility.
  • Monitor medication, vaccine and anesthesia drug supply; including ordering, ensuring proper storage, and reviewing
expiration dates. Ensure medication products and clinical equipment are up to date & available as needed.
  • Serve in a clinical capacity as needed. Understand and support the practice of other team members and refer
patients appropriately.
  • Identify, investigate, and proactively respond to risk management issues in accordance with national and ACPNY
standards.
  • Serve as a clinical liaison between the call center, revenue cycle team and the medical office to promote positive
patient experience.
  • Promote employee engagement and wellness, shared decision making, and collaboration with all members of the
healthcare team. Develop staff related initiatives to show gratitude and celebrate staff recognition.
  • Perform other job-related duties as required.
Education, Training, Licenses, Certifications
  • Bachelor's Degree in Nursing (R)
  • Associate Degree in Nursing plus additional experience/specialized training may be considered in lieu of Bachelor's Degree
requirement. (R)
  • Current, unencumbered New York State Registered Nurse license (R)
  • BLS (Basic Life Support) Certification (R)
  • ACLS (Advanced Cardiovascular Life Support) Certification (P) (Required for offices with active GI suites)
Relevant Work Experience, Knowledge, Skills, And Abilities
  • Minimum of five years' experience as a Registered Nurse (R)
  • Minimum of two years of supervisory or teaching experience (R)
  • Outpatient ambulatory care experience (P)
  • Proficient in the use of Electronic Medical Record (EMR) (R)
  • Proficient with MS Office (Word, Excel, PowerPoint, Outlook, Teams, etc.) (R)
  • Detail oriented; strong analytical and problem-solving skills; ability to execute on strategy. (R)
  • Ability to organize, prioritize, and effectively manage multiple tasks with competing priority levels and deadlines. (R)
  • Excellent communication skills (verbal, written, presentation, interpersonal) with all types/levels of audiences. (R)
  • Able to analyze problems and recommend and deploy strategies to improve performance in collaboration with the leadership
