Enrich thelives of older adults through community, choice, and continuing care in theManager of Home Care role. In this role, you willprovide direct supervision of home care services for an assigned territoryfocused on growing the volume of services in private homes. The primary role of the Manager of Home Careis to ensure residents contracted to receive home care services have theappropriate level of care and care plan, and the home care aides deliver safeand appropriate care according to company policy and state regulations. This role will work collaboratively with theDirector of Home Care and the Home Services Liaison to both grow the volume ofresidents served in private homes (off campus) and ensure stability ofservices. You willprovide continuing care through these responsibilities
Essentialfunctions of this position include the following.
Supervision of Client Services(service delivery)
- Responds to inquiries for Home Care services, screens forappropriate level of care and redirects to other Wesley services, asappropriate.
- Completes admission process, including explanation of services andobtaining signed consent and financial agreement.
- Completes client assessment and develops the care plan.
- Facilitates smooth initiation of services, including scheduling,orientation of caregivers, and client response to home care services.
- Completes data entry into the EMR. Maintains complete and timelyclient records.
- Completes on-going evaluation of home care services and clientreassessments every 6 months, or as indicated according to client health statusand requests. Revises the care plan as required.
- Maintains compliance with WAC 246-335 defined for licensed HomeCare. Demonstrates understanding and compliance with regulatory requirements,Wesley organizational policies, and agency policies and procedures.
- Functions as a member of the campus resident team, workingcollaboratively with the Wellness Nurse, campus leadership, social servicesworker, and other resident services to ensure safety and quality of residentexperience. Participates in resident review as assigned.
Supervisionof Home Care Aides (care delivery)
- Responsible for hiring, orientation, training, and on-going evaluationof home care aides' performance.
- Primary person responsible forcompleting scheduling activities.
*Responsible for overseeing carefor approx. 30 or less home care clients .
- Supports program growth by providing gap coverage for residential carethat does not require certification such as pendant calls, escorts, fooddelivery, redirection and problem solving, as required.
- Works within the scope of any active certification or professionallicensure, as applicable.
- Maintains documentation as required.
- Completes quality assurance activities as directed. Receives andreviews incident reports and takes appropriate action, according to policies.Reports to Director as required.
OtherHealth & Home Care Duties
- Participates in marketing activities, especially as relates tomeeting new campus fill targets and growth of Catered Living program.
- Accesses the electronic health record and extracts data fromsystems for reporting, compliance and dashboard indicators.
- Provides coverage on Wesley campuses as assigned, such as forscheduled or unscheduled absences of the campus Home Care Manager II or specialevents.
- Other duties and projects as assigned.
Our requirements and qualifications for success
- High school diploma or GED required; Bachelor'sdegree in Marketing, Business Administration, Social Services or related fieldpreferred
- Valid CPR and First Aid certification
- Valid Washington State Driver's Licenseand active automobile insurance
- Washington State Certified NursingAssistant license a plus (NA-C)
- Demonstrated proficiency in providing direct supervision ofcaregiver staff.
- Demonstrated ability to complete home care clientassessments/reassessments and care plan development.
- Competence in use of technology, data entry into EMR, computerskills (email, MS Word, Excel, other)
- Superior written, verbal and interpersonal communication skills.Ability to communicate effectively 1:1 and in group settings.
- Professional, organized, detail-oriented and resident/customerservice oriented.
- Ability to appropriately handle confidential information andmaintain confidences.
- Minimum two years' experience in Home Care or similar supervisoryor management role.
- Experience in back-office operations to include scheduling, datamanagement, and reporting.
- Demonstrated interest and/or experience interacting with olderadults and their family members.
Physical,environmental, and mental requirements
- Ability to stand/walk/bend/twist for up to 8 hours per day.
- Ability to sit for periods for record review, phone consulting,and computer data entry.
