The Arc of the Farmington Valley, Inc., more commonly known as Favarh, was founded in 1958 by eight local families who all wanted a better life for their children with intellectual disabilities. A small school was created in the basement of a local church because children with intellectual disabilities were not accepted into public schools at that time. Today, Favarh (pronounced FAY-var ) is a local chapter of the Arc, the world's largest community-based organization for people with intellectual, physical and developmental disabilities. Favarh supports more than 350 adolescents, young adults, adults, seniors and families throughout the Greater Farmington Valley area and beyond.We have more than 330 employees making us the largest employer in Canton. Our more than 100 active volunteers work in our Thrift Shop, participate in our Unified Sports Teams, participate in our creative arts and unified activities, and participate in fundraising efforts. Many of our employees freely choose to volunteer in one or more of these areas demonstrating the level of commitment and dedication to the mission and purpose of the agency. Some of our employees even started out as volunteers and some of our former employees continue to volunteer after they move on.Our mission is To Help Each Person Achieve Their Personal Best as we encourage and support a greater level of independence and personal satisfaction for each person that we support.The Supportive Housing Manager (SHM) reports to the Supportive Housing Coordinator, shall function in accordance with Favarh policies as established by the Board of Directors. The SHM is responsible for the daily operation that will enhance opportunities for our participants in housing complexes in the Farmington Valley, CT. This represents the third complex to bring affordable, inclusive living to individuals with intellectual disabilities. RESPONSIBILITIES:
  • Operations:
    • Participates in interview/hiring process of program staff as requested; implements staff disciplinary action with approval from Supportive Housing Coordinator
    • Assists the Supportive Housing Coordinator/Director in the intake/habilitation/discharge process for participants.
    • Provides 24-hour, on-call coverage, as specified in Emergency On-Call Procedures.
    • Directly supervises, trains and mentors on-site Supportive Housing Assistant Manager, Medical Liaison and Direct Support Professionals
    • Ensures staff instruction and follow regarding condition of individual's apartments as necessary.
    • Ensures that participants possess needed personal items, i.e., clothing and hygiene supplies, etc. and provides individual counseling to participants for minor adjustment programs, as needed, referring serious problems to the supportive housing coordinator.
  • Communication:
    • Possess and demonstrates effective interpersonal skills, which enhance communication between participants and/or staff. Available for a flexible work schedule within 7-day week
    • Coordinates the participant's and his/her family's orientation to Favarh.
    • Utilizes direct support staff and Interdisciplinary Team (IDT) input to identify and prioritize participant's needs and goals in the development and implementation of the IP.
    • Develops and maintains good public relations and publicity by interpreting continuously to neighbors, participants' families, local community groups, the purpose and needs of the Supportive Housing Department and Favarh.
    • Serves as designated liaison between physician and participant, as needed:
    • Ensures scheduling of all medical appointments with licensing regulations (annual physicals, psychiatric treatment, dental, follow-up appointments, etc.)
  • Policy &Procedures:
    • Upon acceptance into a residential program, coordinates/completes documentation required for transition to the site. Establish a program participant file at the Main Office and on Residential Site.
  • Follows procedures in a manner that respects the human and civil rights of individual participants and staff.
      • Ensures reporting, in writing and in person, on the progress and medical/medication issues of each participant at quarterly reviews.
      • Monitors all participants' financial transactions and follows established financial procedures.
      • Ensures the timely completion of the Aquatic Screening, the LON and the Life Skills Assessment annually.
      • Ensures all required documentation is obtained and filed in situations where families are overseeing medical appointments.
      • Represents participants at DDS North Region's Program Review Committee/Human Rights Committee as assigned and coordinates with the Favarh Behaviorist and participant's psychiatrist/physicians to implement PRC/HRC recommendations.
      • Purchases/orders program supplies through Amazon/Purchasing with approval from Coordinator and Director of Supportive Housing.
      • Ensures billable services are within the individuals' allocations and that EVV matches Therap documentation. Works with Supportive Housing Coordinator, Director of Supportive Housing and team on monthly attendance.
      • Implements safety procedures, safety training and development of safe teaching techniques for all staff/participants on site.
      • Follows Favarh Van Driver Policy/Procedure. Provides or/and ensures transportation of participants, as assigned, or as needed.
    • Training:
      • Coordinates staff orientation and training as required by Favarh & DDS REPLACE WITH QUALITY ASSURANCE Regulations and assures on the floor training to Direct Support Professionals.
      • Provides training/materials for staff so that they may initiate and supervise leisure activities for participants during leisure periods.
    • Technology:
    • Utilizes Therap for all program related documentation and ensures that staff is trained in its use and that it is utilized by staff in accordance to established policies and procedures.
    • Develops/monitors Therap, communication, nursing, medication, behavior and other individual documentation, as required
    QUALIFICATIONS:
    • Bachelor's Degree preferred or equivalent experience within the IDD arena.
    • Certifications: PMT, First Aid, CPR and AED
    • Medication Administration Certificate required within 3 months of hire to continue position.
    • Must complete all required DDS/Favarh required training
    • Possess a State of CT Drivers License with clean driving record, to pass Ride Share approval for transport
