Serves as case manager for identified consumers Essential Functions

May determine consumer eligibility to participate in home-based case management services;

Assists older adults and persons with disabilities in obtaining access to and qualifying for social, financial, and community benefits;

Provides guidance to seniors and families through MAP Senior Information Assistance and State Health Insurance Assistance programs;

Makes referrals, organizes and schedules special assistance and benefits programs for older adults;

Maintains appropriate statistical data and case records;

Establishes and maintains good relationships with other relevant community service agencies;

May visit homes, hospitals, etc., as necessary;

Conducts outreach and public education events;

Performs other duties as assigned.

Required Knowledge, Skills, and Abilities

Ability to gain thorough knowledge of St. Mary's County Government policies and procedures;

Ability to effectively communicate with staff, consumers, their families, other community agencies, and the public in a courteous manner;

Thorough knowledge of government and community programs, services, and resources available to senior citizens;

Ability to work independently on tasks and maintain records;

Ability to plan, organize, and execute essential tasks with minimum instruction or review.

Education and Experience

Bachelor's Degree in a related field;

Or equivalent technical training, education, and/or experience.

Physical and Environmental Conditions:

Work requires no unusual demand for physical effort.

Work environment involves moderate risks or discomforts which require safety precautions when visiting consumers' homes.
