Serves as case manager for identified consumers Essential Functions
May determine consumer eligibility to participate in home-based case management services;Assists older adults and persons with disabilities in obtaining access to and qualifying for social, financial, and community benefits;Provides guidance to seniors and families through MAP Senior Information Assistance and State Health Insurance Assistance programs;Makes referrals, organizes and schedules special assistance and benefits programs for older adults;Maintains appropriate statistical data and case records;Establishes and maintains good relationships with other relevant community service agencies;May visit homes, hospitals, etc., as necessary;Conducts outreach and public education events;Performs other duties as assigned. Required Knowledge, Skills, and Abilities Ability to gain thorough knowledge of St. Mary's County Government policies and procedures;Ability to effectively communicate with staff, consumers, their families, other community agencies, and the public in a courteous manner;Thorough knowledge of government and community programs, services, and resources available to senior citizens;Ability to work independently on tasks and maintain records;Ability to plan, organize, and execute essential tasks with minimum instruction or review.
