Must meet current licensure and/or certification requirements in state(s) and cities of practice.2. If practicing in an unregulated state, must be a graduate of an accredited Massage Therapy program with a minimum of 600 hours of training in Massage Therapy. (Certified Massage Therapists).3. Completion of or eligible to sit for Board Certification Exam (BCETMB) or MBLex.4. Membership in a professional organization (AMTA or ABMP) preferred.5. Meets personnel qualifications stated in 418.114(b)(8) CMS regulatory requirements.6. Ability to pass DHS background study.7. If Driving: Valid State Drivers License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
