We are looking for a dedicated Registered Nurse Assessment Coordinator (LPN/RN), also known as an MDS Coordinator, to join our skilled nursing facility near Gettysburg, PA.Position Overview: This role is focused on conducting and coordinating resident assessments, ensuring compliance with state regulations, and aligning with the facility's goals.Available Shift:
- Full-Time, Monday to Friday
Key Responsibilities:
- Perform routine resident assessments based on pre-determined schedules or as needed due to condition changes, hospital returns, and other situations.
- Accurately observe, assess, and communicate changes in resident conditions to relevant staff.
- Ensure timely completion of assessments and CAA's by other departments (Social Services, Activities, Dietary) and obtain necessary signatures.
- Complete other MDS sections, quarterly reviews, CAAs, and care plans when required.
- Develop care plans based on resident needs from Resident Assessment Protocols, and update them as necessary.
- Prepare and distribute the MDS schedule to the interdisciplinary team.
- Transmit MDS data to the state on a weekly basis (or as required) and maintain accurate transmittal logs.
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
- At least one year of experience as an MDS Assessment Coordinator in a long-term care setting
