MDS Assessor Nurse (Registered Nurse)

Duties and Responsibilities of the MDS Assessor Nurse (Registered Nurse):

  • Initiating care plans and supporting activities as assigned
  • Creating and distributing monthly care plan calendars in a timely fashion
  • Maintaining & updating all care plans and assessments as required
  • Complete nursing assessments and all other nursing components correlating to the MDS.
  • Follow up on bloodwork, IV orders, and administration.
  • Work collaboratively with the MDS vendor team on all aspects of the MDS process. (Including any potential audits).
  • Join and update the nursing components of the weekly utilization review meetings.

Requirements of the MDS Assessor Nurse (Registered Nurse):

  • Must have prior MDS experience in a long term care setting.
  • Valid NYS Registered Nurse (RN) License.
  • Must have a current/active CPR certification.
  • Excellent technical, assessment, and documentation skills.
  • Effective verbal and written English communication skills.
  • Highest level of professionalism with the ability to maintain confidentiality.
