We are seeking a compassionate and detail-oriented MDS Coordinator to join our Nursing Management team at Eastcastle Place. In this role, you will oversee the quality, accuracy, and timeliness of resident assessments and care planning in accordance with Federal, State, and local regulations. You will collaborate with a dedicated team, including Social Workers, Admissions, Nurse Managers, CNAs, RNs, and LPNs, and report directly to our Director of Nursing. What You Will Do:
  • Oversee and coordinate the MDS process for resident assessments and care plans.
  • Ensure assessments are completed accurately and within regulatory timelines.
  • Collaborate with interdisciplinary team members to implement comprehensive care plans.
  • Ensure timely physician certification and accurate diagnosis code entry.
  • Optimize reimbursements in collaboration with the Interdisciplinary Team (IDT).
  • Monitor compliance with state and federal regulations, providing guidance and training.
  • Conduct audits and quality assurance reviews to uphold care standards.
  • Serve as a resource for staff on MDS-related questions and issues.
  • Facilitate ongoing education and training regarding MDS processes and updates.
  • Participate in care conferences, communicating effectively with residents, families, and healthcare providers.
  • Maintain resident records per regulations, utilizing electronic charting systems.
  • Assess potential resident admissions and collaborate with discharge planners.
  • Report admissions to nursing staff and discuss appropriateness with the Director of Nursing.
  • Participate in nursing management, monitor staffing, and conduct staff meetings.
  • Join the Nurse On Call rotation after 90 days; oversee scheduling and immediate needs outside regular hours.
What You Will Bring:
  • Minimum of 2 years of MDS completion experience.
  • Current RN license in the state of Wisconsin.
  • Exceptional organizational skills and attention to detail.
  • Strong time management and goal-setting abilities.
  • Ability to work collaboratively in a team environment.
  • Proficient understanding of State and Federal guidelines regarding nursing and care delivery.
  • Experience in performance management.
  • Strong assessment skills and clinical knowledge.
  • A passion for resident care, empathy, and leadership skills.
  • Willingness to learn and strong active listening skills.
