MDS Coordinator/Case Manager

\*\*Job Summary:\*\* We are seeking a detail-oriented MDS Coordinator to join our healthcare team. The MDS Coordinator will be responsible for ensuring accurate and timely completion of resident assessments in compliance with all applicable regulations. \*\*Duties:\*\* - Conduct comprehensive assessments of residents' care needs - Coordinate with healthcare providers to develop individualized care plans - Review and analyze medical documentation to ensure accuracy and completeness - Utilize knowledge of HIPAA regulations to maintain confidentiality of resident information - Collaborate with nursing staff on discharge planning and transitions of care - Oversee the ICD-9 coding process for medical diagnoses - Monitor and update resident records regularly - Assist in the coordination of medical management for residents in acute and primary care settings \*\*Requirements:\*\* - Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience in an acute care, primary care, or nursing home setting - Proficiency in documentation review and medical documentation processes - Familiarity with ICD-9 coding standards - Strong understanding of discharge planning procedures - Excellent communication and interpersonal skills - Ability to work collaboratively in a multidisciplinary team environment This position offers a competitive salary, benefits package, and opportunities for professional growth.
