Job Description:

The MDS Coordinator continuously monitors, evaluates and manages the care given to residents to ensure effectiveness of the care plans and that all strategies comply with all federal, state and ethical standards. Uses a resident management system and assessments to gather information from residents and their families during initial and periodic interviews covering such areas as the patient's mood, behavior patterns, cognitive ability and nutrition needs to develop a care plan that has long-term and short-term goals for resident improvement. Works collaboratively to support the Mission and Values of SCBMA. Requirements Essential Duties and Responsibilities:

  • Continuously evaluate residents (Medicare/Medicaid) to determine if goals are reached and if the resident remains eligible for their current services or changes in the care plan
  • Screen all potential admissions for Medicare/Medicaid eligibility
  • Coordinate and conduct Welcome care plan meetings for new admissions
  • Work with Business Office and Rehabilitation Department to validate reimbursements from Medicare, Medicaid, VA or other entities
  • Serve as chairperson/nursing coordinator of the interdisciplinary assessment and care planning team to discuss goals and plans of action and to inform the resident and/or family or responsible party of goals and plans of action
  • Work closely with nursing and therapy departments to coordinate Medicare skilled services
  • Conduct weekly meetings with all Healthcare staff regarding rehabilitation residents for progress and next steps to ensure patients receive ability to return to a safe environment when discharged
  • Ensure accuracy of medical records and coding procedures. Prepare and provide reports to regulatory agencies and the Director of Nursing in a timely manner as required
  • Submit MDS data to CMS website timely as required
  • Prepare a quarterly quality indicator report for the Director of Nursing using data from the CMS site to monitor alerts and needed action
  • Maintain required forms and documentation related to health care surveys
  • Maintain up to date knowledge of federal and state regulations, as related to the position, particularly Medicare and Medicaid programs.
  • Ensure compliance with all regulations from DHEC, CMS, Fire and Safety, OSHA, Labor Laws, etc and adhere to HIPAA confidentiality standards.
