Napa Post Acute is seeking a skilled and experienced MDS Coordinator (LVN or RN) to join our dedicated team in Napa, CA. Responsibilities:
  • Conduct comprehensive MDS assessments in compliance with state and federal regulations.
  • Develop and update individualized care plans based on thorough assessments and interdisciplinary team input.
  • Ensure accurate and timely documentation to support optimal reimbursement and compliance.
  • Collaborate with nursing staff, therapists, and other healthcare professionals to deliver exceptional resident care.
  • Participate in care plan meetings and quality improvement initiatives to enhance resident outcomes.
  • Monitor MDS processes to ensure consistency, accuracy, and adherence to all regulatory requirements.
Qualifications:
  • Current California LVN or RN license.
  • Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting.
  • In-depth knowledge of MDS 3.0, care planning, and regulatory guidelines.
  • Strong organizational skills with a keen attention to detail.
  • Excellent communication and teamwork abilities.
