Here's what you will do: *Work in collaboration with facility team to assess the needs of the resident *Assist with coordination and management of care management meetings, to include review of resident care and setting of assessment reference dates *Comply with federal and state regulations regarding completion and coordination of the MDS process *Monitor MDS and care plan documentation for all residents *Ensure documentation is present in the medical record to support MDS coding *Maintain the frequent and accurate data entry of resident information into appropriate computerized program *Complete accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members *As required, attend team meetings and other meetings in order to gather information, communicate changes, and maintain and update records *Continually update knowledge base related to data entry and computer technology *May participate in quality assurance activities as required *May complete electronic submission of required documentation to required databases and other entities per company policy *Perform additional duties as assigned Essential For You to Have as a Campbell Street Senior Living MDS Coordinator: *RN licensure within the state of Iowa, plus a minimum of two years MDS experience in long-term care (Prefer RN but will consider LPN) may be considered with exceptional qualifications or equivalent combination of education and experience. *Must have current AANAC (RAC-CT) certification or obtain it within 90 days of hire *Have and continue acceptable Criminal Background report *Have and continue clean Sex Offender report *Have and continue clean Child and Adult Protective Service report *Understand, speak, read, and write English fluently *Education, experience, and skills to perform required responsibilities *Ability to treat and care for our residents and their property and personal belongings with dignity and respect Know someone who'd be great for any of our CSSL roles?
