MDS Coordinator - Skilled Nursing Community

We are seeking an outstanding Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at a large senior living community in Grand Junction, CO. Job Description

The MDS Coordinator is responsible for the overall coordination of the facility Medicare, Resident Assessment Instrument (MDS) and care planning process.

Responsibilities

  • Assess and evaluate the quality of care being given to long-term care residents
  • Perform the initial interview and assess new residents
  • Develop a care plan that has long-term and short-term goals for improvement
  • Conduct meetings with multidisciplinary treatment teams, caretakers, and families
  • Reach out to specialists within the community to get help in achieving care plan goals

Qualifications

  • Graduate of accredited nursing program
  • Must have a valid, unrestricted LPN or RN license in the state of Colorado
  • Previous MDS experience preferred
  • Previous experience in Long Term preferred, but not required
  • PCC knowledge preferred
