Responsibilities:
- Schedules residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, overseeing communication to nursing and other departments.
- Assures compliance with Federal and State regulations regarding completion/coordination of the comprehensive assessment (MDS).
- Monitors MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file.
- Maintains proficiency in software.
- Coordinate and attends regular Medicaid meetings to discuss current residents, pending residents and recipients.
- Coordinate and attends all ITTM meetings in order to gather information, communicate changes, maintain and update the clinical records including the Care plan.
- Performs care plan updates as needed.
- Provide DON, Administrator and pertinent staff with the Quality Indicators Report.
- Perform transmissions of the MDS on a timely basis.
- Remains current with changes in the industry through attendance at outside workshops and conferences.
- Perform Ancillary management, collection and reporting to corporate billing personnel.
Requirements:
- Licensed Nurse in good standing.
- Geriatric nursing background.
- Restorative care philosophy, excellent professional skill (both assessment and hands-on and good communication and organization skills).
- Working knowledge of nursing home regulations.
- Ability to work independently as well as a meaningful member of the team.
- Excellent organizational skills in order to coordinate many projects simultaneously.
