As an MDS Coordinator, you will play a pivotal role in ensuring the quality of care and compliance with federal and state regulations. Compensation & Benefits:

Salary: $40/hr

$10,000 Sign-On Bonus for qualified candidates

Shifts:

Monday - Friday, 8:00 am - 5:00 pm

Key Responsibilities:

Conduct comprehensive assessments of new and existing residents using the MDS tool.

Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.

Ensure timely transmission of MDS assessments to relevant authorities.

Train and guide nursing staff on MDS procedures and updates.

Monitor and audit clinical records for accuracy and completeness.

Collaborate with healthcare providers to optimize patient care and outcomes.

Qualifications:

Active RN license with MDS Certification

Proven experience in clinical assessment and MDS coordination.

In-depth knowledge of federal and state regulations related to MDS and care planning.

Strong organizational skills and attention to detail.

Excellent communication and interpersonal skills to interact with staff at all levels.
