About the Position:

MVP Recruitment, LLC is seeking a skilled RN MDS Coordinator for a 120-bed Skilled Nursing Center located near Fitchburg, MA. This role is pivotal in managing the care and assessment process for short-term and skilled patients within a supportive and well-staffed environment. This position is fully on-site and does not involve any on-call responsibilities.

Position Overview:

As the RN MDS Coordinator, you will be responsible for conducting and overseeing the MDS assessment process, with a focus on short-term and skilled patients. You will work closely with a dedicated team that includes a full-time RN Coordinator, a full-time MDS Nurse, a part-time Nurse who assists with interviews, and a full-time Medicaid Nurse. Your primary role will be to ensure accurate and timely assessments, fostering positive outcomes for patients while adhering to all relevant regulations.

Key Responsibilities:

  • Conduct and coordinate the completion of resident assessments (MDS) for short-term/skilled patients.
  • Collaborate with the interdisciplinary team to develop and update comprehensive care plans.
  • Ensure compliance with state and federal regulations regarding MDS assessments and documentation.
  • Work closely with the office team to manage patient assessments and care planning effectively.
  • Focus on delivering high-quality care and achieving optimal patient outcomes.

Qualifications:

  • Current RN license in the state of Massachusetts.
  • Minimum of 2 years of experience in MDS coordination, particularly with short-term/skilled patients.
