Location: Ambassador Healthcare at College ParkPosition Type: Full-Time, 8:00 AM – 4:00 PMPay Rate: $38–$42 per hour

Job Responsibilities:
- Oversee the development and completion of the resident assessment (MDS) following current rules, regulations, and guidelines.
- Implement RAPs, Triggers, and comprehensive care plans in collaboration with the Interdisciplinary Care Plan Team.
- Maintain and update policies governing resident assessments and care plans.
- Coordinate quality assurance programs related to resident assessments and care plans.
- Assist residents and the Discharge Planning Coordinator in creating discharge plans.
- Prepare for and participate in facility surveys and inspections.
- Monitor QI and QM reports, recommending corrective actions for potential problem areas.
- Ensure timely completion and transmission of assessments, reporting issues to the Administrator.
- Collaborate with department supervisors, healthcare professionals, and external care plan team members.
- Evaluate residents' abilities and limitations, contributing to treatment and activity planning.
- Support therapy scheduling and implementation in collaboration with occupational therapists.
Requirements:
- Licensure: LPN or RN
- Experience: Minimum of 1 year as an MDS Coordinator in a nursing home setting.
