MDS COORDINATOR - PART-TIMEPosition Summary:For over 35 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. The Minimum Date Set (MDS) Coordinator works with clinical and all other staff to ensure care plan guidelines for each Resident are followed, and compliant with ethical standards and Medicare requirements.Job Specific Duties:Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Maintains a schedule of MDS assessment dates. Inputs and transmits MDS timely to meet regulatory requirementsMonitors projected versus actual PDPM classification for each Med A, or Medicaid Resident. Works collaboratively with interdisciplinary team, rehabilitation and business office to maximize reimbursement appropriatelyWorks to complete MMQ assessmentsMonitors compliance of the Interdisciplinary Team in meeting MDS submission deadlines to ensure compliance with regulationsParticipates in the analysis of the quality indicator data along with the DNS, Nursing Supervisors, and other Team Members to identify actual or potential quality issuesDocuments pertinent findings and interventions in the appropriate care planAssists in orienting new Team MembersConducts trainings for appropriate team members as neededServes on various committees of the skilled nursing facility (i.e., QAPI, Care Planning, etc.)Identifies possible QAPI initiatives via statistical data gathered and monthly QI indicator reports when availableMakes recommendations regarding QI activities to effect changes in North Hill operations and assure compliance with regulatory requirements when availableRecognizes and reports all allegations of Resident abuse, neglect, exploitation, social media exposure, and/or misappropriation of Resident property immediatelyParticipates in coordinating the interdisciplinary team assessment of each Resident quarterly and as neededDocuments pertinent findings in the CAA notes and ensure that other IDC team members document their pertinent findings in assigned notesPrepares a concise quarterly note for each Resident and/or responsible party that details the Resident's overall status over the last three monthsPerforms informal chart audits with each MDS assessmentPromotes participation by the resident whenever feasible in the development of the ICPCoordinates the completion of facility specific assessment formsPerforms AIMS testing semi-annually as neededReviews consultant notes during the assessment period for areas of concern and addresses them with the licensed team member for interventions and informs the DNS of concernsLooks up and reports to the team all proper ICD-10 codes to utilize on all admissions prior to admissionPerforms triple check with Billing and RehabInitiates and ensures completion of all Medicare Certifications by MD/NPAssigns quarterly assessments and ensures they are completed by the household nursesMeets department uniform, appearance and grooming requirementsComplies with all OSHA safety regulations, local applicable laws regarding health, safety or environmentEducation and Experience:A minimum, 3 year(s) of experience in long-term care setting, or other related health care facility.Must have working knowledge of MDS/RAI process and OBRA guidelines.Must possess a active license to practice as a RN in MassachusettsMust maintain first aid /CPR certificationMust be proficient with ICD-10 CodingMust be knowledgeable in Triple Check Practices and PDPMMust be proficient in creating resident specific care plansProficient with computer software programs and EMRNorth Hill offers:Low patient ratioFree mealsCompetitive shift differentialFree parkingFree on-site fitness centerState-of-the-art work environmentEmployee referral programHealth & Dental Benefits401(k) with company matchCareer Development TrainingTuition ReimbursementNorth Hill is committed to providing equal opportunity for employment and advancement to all team members and potential team members.
