Your Role as theMDS Coordinator RN: Americare SeniorLiving is seeking a dedicated MDS Coordinator RN to coordinate MDS assessmentsper federal guidelines. In this role, you will ensure residents receive qualitycare and a home-like environment while supporting their families. You will alsohelp maintain facility standards and assist the Director of Nursing withinspections, care planning, and staff support. What You'll Do:
  • Coordinate and maintain all required resident documentation, including Nursing Assessments, Care Plans, and Notes, ensuring compliance with ASI, state, and federal regulations
  • Assist in keeping facility standards up to state and federal requirements and help with inspections
  • Evaluate medication systems for effectiveness and compliance with state regulations
  • Communicate significant changes in resident conditions to the administrator and ensure proper reporting
  • Assist with resident admissions, transfers, and decisions on care placement
  • Conduct nursing staff meetings, assess equipment needs, and ensure sufficient supplies
  • Complete and update assessments, care plans, and MDS sections as required, while participating in Medicare and care plan meetings
  • Adhere to the Americare Mission/Vision and uphold our company values
Physical Demands: This positionrequires standing, sitting, walking, lifting up to 25 pounds and using equipmentfor resident care. Safety procedures, including two-man lifts and the use ofmechanical lifts, are essential for ensuring the safety of both staff andresidents. Qualifications:
  • Graduate of an accredited school of nursing (RN)
  • Current Registered Nurse license
  • Experience with MDS 3.0 and skilled nursing required
  • Familiarity with Medicare, state, and federal regulations
  • Preferred: 1+ years of clinical experience in skilled nursing facilities and 2+ years in long-term care
