Up to $15,000 Sign on Bonus*Job Title: MDS Coordinator Department: Seneca Hill Manor - Nursing Reports to: Director of Nursing Revision Date: 08/14/2024 Grade Level: 32 Status: Exempt Job Summary: The MDS Coordinator is responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Coordinator will coordinate the Resident Assessment Instrument (RAI) process assuring the timeliness, and completeness of the Minimum Data Set (MDS), CAA's, and interdisciplinary care plans. Duties/Responsibilities:
  • Ensures that the RAI and the documentation to support the RAI for regulatory and reimbursement purposes are within the standards of Principle Long Term Care and Federal and State guidelines
  • Participates in the training and education of interdisciplinary team members in all areas of the RAI process, and quality measures
  • Completes initial, quarterly, annual, significant change, and special Minimum Data Set (MDS) and care plans in accordance with State and Federal guidelines
  • Ensures the timely electronic submission of all MDS to facility's chosen scrubber
  • Ensures the timely electronic submission of all MDS to iQIES
  • Accurately reports resident conditions and services provided in accordance with Federal and State guidelines in an effort to maximize quality measure ratings, and reimbursement through all payer sources
  • Understands and maximizes CMI
  • Responsible for the education of the skilled nursing staff on MDS processes and documentation requirements
  • Partners with departments, including therapy and finance, to ensure accurate and timely billing for areas related to the MDS
  • Performs medical record review as requested by CMS/HMO's using their guidelines
  • Schedules and conducts weekly care plans
  • Attending interdisciplinary team meetings, as appropriate
  • Develops an appropriate care pathway and timely communication of needed information to the resident, family, other health care professionals and third party payers
  • Selects the correct reason for assessment and Assessment Reference Date (ARD)
  • Reviews state validation reports and ensures that appropriate follow-up action is taken
  • Schedules MDS to maximize reimbursement
  • Completes accurate coding of the MDS with information obtained via medical record review observation and interview with facility staff, resident, and family members
  • Follows Seneca Hill Manor facility policies/procedures as they pertain to specific duties.
  • Maintains MDS and care conference schedules
  • Completes assigned assessments to facilitate MDS completion
  • Checks hospital records and MD/NP documentation on website for pertinent information regarding MDS
  • Continuous exposure to patients, families and general public in addition to general office environment
  • Performs other duties as requested.
Required Skills/Abilities (Unit Specific):
  • Ability to supervise, direct, and evaluate the nursing staff in the provision of resident care and performance of job duties.
  • Demonstrated effective communication, conflict resolution and problem solving skills.
  • Knowledge of the process of aging as it relates to health care as well as resident rights and abuse regulations.
  • Knowledge in determining the type and amount of nursing staff and supplies needed to provide quality resident care.
  • Demonstrate correct and safe technique in use of all nursing/resident equipment.
  • Ability to communicate effectively, both orally and in writing.
  • Professionalism and business acumen appropriate for working with guests, clients, physicians, employees and volunteers.
  • Excellent time management skills, punctuality and a sense of urgency in completing tasks.
  • Ability to cost effectively manage resources and supplies.
Education and Experience:
  • Must have a current license to practice as RN in New York State. One (1) year of skilled nursing facility, long term care experience, rehabilitation nursing or other healthcare related facility and previous experience in completing Minimum Data Set (MDS), Resident Assessment Instrument (RAI) process and/or Care Plans. Maintain current RAC-CT certification
Working Conditions:
  • Requires attendance of seminars, workshops, conferences when appropriate.
  • Actively participates in in-service programs (12 hours annually), in Quality Assurance programs and CQI, cross training; attends all mandatory classes as required; completes unit-specific competencies.
  • Requires attendance of various meetings and participation on committees as required.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15lbs at times.
  • Occasionally requires assignments to other units/shifts as hospital needs require.
  • Complies with DOH requirements for annual health assessments and required immunizations.
*Sign on for full time positions Pay Range:$85,500-$110,000/annual Employee salary is based on commensurate experience and other qualifications.
