As an MDS Coordinator, you will be at the heart of resident care, ensuring accurate assessments and individualized care plans. You'll be responsible for coordinating the Minimum Data Set (MDS) process, crucial for care planning and compliance. Your role directly impacts the quality of care and financial reimbursement, making it essential to both residents' well-being and the facility's success. Responsibilities:

  • Oversee and manage the entire MDS process, ensuring all assessments are completed accurately and on schedule to comply with federal and state regulations.
  • Perform initial, quarterly, annual, and significant change assessments, maintaining high standards of accuracy in resident documentation.
  • Develop, review, and adjust individualized care plans based on resident assessments, collaborating closely with the interdisciplinary team, including nursing, therapy, social services, and dietary departments.
  • Analyze MDS data to optimize care planning and improve quality measures; ensure data accuracy for reimbursement and compliance audits.
  • Lead and participate in resident care conferences, fostering clear communication between staff, residents, and families.
  • Conduct ongoing training and education for nursing and interdisciplinary staff on MDS requirements, coding, and best practices.
  • Ensure timely submission of MDS data to state and federal agencies, monitoring for errors and correcting submissions when necessary.
  • Maintain up-to-date knowledge of MDS guidelines, including changes in regulations, reimbursement models, and quality measures.
Qualifications:
  • Active RN or LPN license in New York State, with a preference for candidates with AANAC certification (Resident Assessment Coordinator Certified).
  • Minimum 2-3 years of experience as an MDS Coordinator in a skilled nursing facility, with comprehensive knowledge of MDS 3.0, care planning, and reimbursement processes (PPS, PDPM).
  • Strong understanding of federal and state regulations governing MDS and skilled nursing facility operations.
  • Exceptional organizational skills, with the ability to manage multiple assessments and meet tight deadlines consistently.
  • Proficient in electronic health records (EHR) systems and MDS software; strong analytical skills for data review and quality improvement initiatives.
  • Strong interpersonal and communication skills, with a collaborative approach to working with multidisciplinary teams.
  • Attention to detail and accuracy, especially in coding and documentation.
Compensation:
  • Competitive salary range of $110,000 - $120,000, with additional compensation for highly qualified candidates.
