The House Calls Medical Assistant/CIS will fulfill a crucial role, supporting logistics, clinical care, and documentation in our innovative home-based care program. The Home Visit Medical Assistant/CIS will work directly with a House Calls nurse practitioner on a daily basis, in collaboration with center-based care teams, to help identify and bridge gaps in care when patients return from the hospital or experience other challenges.Core functions:
- Preparing visit schedule, route, and logistics for all visits
- Accompanying nurse practitioner on all house calls
- Vital signs, medication reconciliation, patient / family education
- Documenting House Call note, managing referrals, orders and follow-up
- Maintaining supplies/equipment
- Other duties as assigned

Requirements:
- At least one year experience as a Medical Assistant + MA certification
- Must pass all clinical competencies before supporting patient care, phlebotomy a plus
- Daily access to a vehicle with valid license and insurance
- Flexibility to travel around Chicagoland
- Bilingual (English and Spanish) a plus
- US work authorization
