JOB SUMMARY: The Medical Care Worker is responsible for the day to day management of medical/clinical services provided to the residents within residences operated by Independence Residences, Inc. The MCW is also responsible for the provision of direct support services to the residents as needed. RESPONSIBILITIES: Attend monthly staff meetings, IDT and MCW Meetings as required. Report upcoming MARs at least 3 days and print. Follow policies and procedures set forth in the residential policy and procedure manual. Maintain a current and up to date medical appointment calendar in the residential facility. Ensure all upcoming and scheduled appointments are logged immediately on the appointment calendar. And report all psychotropic changes to ABS. Schedule and track all medical appointments including follow-ups as needed or recommended within designated time frame. Keep van clean after use and report issues immediately to supervisor. Accompany individual(s) to and from appointments unless otherwise instructed. Advocate and act as a liaison while in the community. Prepare clinic forms and attach all medical information prior to the appointment (e.g. appointment card, lab results, and copy of previous clinic form, Medicaid card etc.). Information should be prepared minimally one day in advance. Review clinic forms prior to leaving the clinic/service provider to ensure the doctors recommendation/order is clearly written, and all prescriptions include start and stop dates. Communicate medical findings in an informative, professional manner (e.g. report results to Program Management, and RN). And submit Dr orders to RN for review, then PCP then take to pharmacy Submit clinic visit forms copy to day programs. Communicate all pertinent medical documentation in the communication log. Do weekly medications check of all medications weekly and document. Monitor the medication storage and ensure sufficient medication supply available daily. Ensure that the medication cabinet/area is cleaned and organized and locked daily. Ensure adequate first aid supplies and equipment (blood pressure cuff, thermometer, nail clipper) are functional and operational. Ensure prescription renewals are filled and/or picked up in a timely manner. Ensure lab work/radiology results are obtained and filed in the consumers chart in a true manner. Review all goal data and submit responds to manager. Additional duties as assigned by supervisor. Often the requirement to work overtime is mandatory to ensure that proper personnel coverage is maintained at IRI facilities. Ensure that proper staff ratio is maintained at all times which necessitate a change in your work schedule, and or location. Computer Medical Care Worker must be able to use a computer and able to learn software programs Office Equipment Medical Care Worker are required to operate a fax, copier, scan, and other office equipment. Driving - Medical Care Worker must be able to operate Agency vehicles to transport the participants of the program to different appointments, events and activities. In addition, having and keeping a valid NYS Drivers License at least one year with less than six (6) points in the past three (3) years is a requirement for this position.
