Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a CBA ) between Housing Works and the RWDSU This position requires applicants to successfully complete a background check through the New York State Office of Children and Family Services when it supports the mandate under Section 424-a of the Social Services Law for legally authorized agencies to inquire of the Statewide Central Register of Child Abuse and Maltreatment (SCR) as to the existence of any indicated reports of child abuse and maltreatment against an applicant prior to employment, certification, or licensure in the childcare field. All background checks will be conducted and reviewed in accordance with applicable federal, state and local law. Compensation Range: $24/hr - $26/hr commensurate with experience.Overview: The Health Services: Retention and Adherence Department seeks a medical case manager to join our team as we address HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approaches to meeting the needs of people living with HIV (PLWH) with team-based care management. The program makes use of a navigation approach to identify, advocate for, and coordinate the complex healthcare and social service systems necessary to ensure improved outcomes among PLWH. Provide comprehensive medical case management services and assess, identify, and address the Social Determinants of Health (SDOH).Responsibilities:
  • Leading comprehensive assessments, organizing case conferences, and ensuring service planning
  • Conduct case-finding activities by working from reports generated from clinic records and reaching out to patients who may have missed an appointment, been out of care, or have high viral loads.
  • Provide comprehensive case management services to PLWH enrolled in the Retention Adherence Program (RAP).
  • Provide intake, assess for Social Determinants of Health (SDOH), needs assessment, HIV orientation, service planning, ongoing reassessment services, and documenting all services in the medical record and AIRS.
  • Provide a client-centered service that links patients with health care, psychological and social services in a manner that ensures timely, coordinated access to medically appropriate levels of care, support services, and continuity of care.
  • Develops linkages and networks with other groups and agencies offering services to PLWH to help promote cooperation and reduce duplication of services among agencies.
  • Provide support and crisis intervention including referrals to emergency services to PLWH.
  • Provide follow-up and outreach for those patients not engaged in care.
  • Provide documentation to reflect patient interaction, outreach attempts service plan updates, and biannual psychosocial assessments.
  • Performs updates and maintenance of patient charts.
  • Provides case management input and psychosocial information concerning patients during medical conferences.
  • Enter case management data into EMR and AIRS databases.
  • Attends conferences and various training sessions, as needed.
  • Refers patients to programs, support groups and medical treatment, as needed.
  • Attend and participate in monthly department, All Staff and other required meetings.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the RAP Policy and Procedure Manual and Employee Handbook.
  • Maintain confidentiality of all aspects of Housing Works Health Services including, but not limited to, patient confidentiality, financials, and employee relations.
Minimum Requirements
  • Bachelor's degree and at least 2 years of case management experience
  • Computer literacy
  • Ability to interact with inter/disciplinary team including medicine, social work, psychiatry, nursing, administration, and other paraprofessionals
  • Ability to work with and understand issues of people with HIV/AIDS, mental illness and substance use populations.
  • Knowledge of and experience with mediation and conflict resolution
  • Good verbal and written communication skills
  • Embraces a harm reduction approach to working with underserved populations.
  • Experience with AIRS and/or eClinical Works is preferred
