Role: This position is a part of the leadership team. Crucial component to the Juniper Village Wellness Staff. The Medical Concierge helps streamline the process and communication flow for primary care providers, ancillary services, therapies, community team, resident and family. Responsibilities: Primary Care Clinic Day Coordination: • Reviews resident schedule and develops list of residents to be seen each day maintaining an efficient work flow of clinic; takes vitals including weight, blood pressure, pulse, temperature, and documents; provides new paperwork/orders to practitioner and assists as needed. Health Quarterbacking/Coaching: • Works 1:1 with residents and/or families to establish self-management goals; schedules appointments and conducts follow-up visits as needed; reminds residents of appointments prior to appointments and coordinates transportation; works with Wellness Team to communicate any resident follow-up needs. Administration: • Sets up new residents and maintains existing residents' electronic files - ensuring all related/required reports, releases, and labs are filed prior to appointments; audits charts monthly to ensure accuracy; reviews and updates insurance information as needed; reviews insurance payment options with residents, families, and providers • Schedules outside appointments as needed, maintains exam room supplies in clinic; triage and processes messages from residents and front office staff for physicians; works as a med tech or LPN as needed. Requirements: • High school diploma • Successful completion of an approved medication administration course or licensed LPN • Successful completion of a CPR and First Aid course (or ability to complete within 30 days of hire) • Knowledge of healthcare field, medical terminology, physiology and psychology of older adults Equal Opportunity Employer #ER51 Please visit our careers page to see more job opportunities.
