(Physician/MD qualifications required) Internal Medicine - Medical Director (FP, IM) Post Acute Care Seeking an experienced Board-Certified Family Medicine or Internal Medicine physician to join a practice providing primary care to adults in skilled nursing facilities. Full-time position Flexible schedule. Set your own schedule and the opportunity to build a position that matches your ideal lifestyle and career goals. No primary night call. Job Duties:Primary care rounding in SNF and Facility Medical Directorship.Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care.Collaborate with clinicians, facility leaders and others regarding care and services.Build relationships with your patients, patients family members, and facility staff to facilitate effective treatment delivery.Provide oversight, guidance, and mentorship to our Nurse Practitioners.Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care.Represent company on appropriate external levels engaging and establishing|maintaining relationships within the post-acute care community.Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues. Estimated encounters per day 15-20 Will not accept visa candidates. Candidates must have at least 1 year post residency or fellowship experience.
