Responsibilities:
* Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
* Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
* Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
* Collaborate with leadership to review care coordination initiatives
* Develop intervention plans, and share ideas about network and risk management issues
* Work alongside pharmacists on medication management
* Implement an individualized plan of care for patients' goals
* Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
* Timely response in chart completions and pharmacy recommendations
* Provide oversight and mentorship to our clinicians in the greater Albany, New York, area
Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply: family (FM), primary care, internal (IM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated range of $215,000 to $242,00 annually with no cap on productivity income potential.
