MINIMUM QUALIFICATIONS

* Valid New York State Driver's License

* Yearly physical (required within one month after hire )

* Ability to work with a diverse population presenting a variety of cultural, socioeconomic, developmental and sexual identities. Must have the ability and willingness to establish and foster healthy and supportive relationships that acknowledge individual identities and encourage personal growth.

PREFERRED QUALIFICATIONS:

* 2-year college degree and or Experience with adolescent teens

REQUIREMENTS

* Must successfully complete all required background checks:

* NYS Statewide Central Register of Child Abuse and Maltreatment (SCR)

* Multi-State Sex Offender Registry (SOR)

* NYS Division of Criminal Justice Services (DCJS) fingerprinting

* NYS Justice Center Staff Exclusion List (SEL)

* NYS OMIG Medicaid Exclusion List

* Out-of-state child abuse register inquiry of any state in which the person resided within the last five years (if applicable)

* NYS License Event Notification Service (LENS)

* Office of Inspector General (OIG)

* Must successfully complete Therapeutic Crisis Intervention (TCI)

HOURS

* 3:00 pm to 11:00 pm; Monday - Friday; flexible to meet the needs of the Program

SALARY

$21.00 per hour

OBJECTIVES OF POSITION

* To assist the nursing staff in providing the highest level of quality care to the residents of the William George Agency.

* To assist the nursing staff in providing safe transport to and from medical appointments off campus.

* To facilitate communication between off-campus medical providers and medical department at the William George Agency.

* To establish fluid lines of communication among in-house personnel, off-campus providers, and medical staff.

* To become an effective member of the treatment team in identifying and meeting the needs of the youth in our care. Communicating needs of each resident to the appropriate medical staff member.

* To assist nursing staff in development of health/safety related issues of the residents and staff.

DUTIES AND RESPONSIBILITIES

* Transport resident to off-campus scheduled appointments as scheduled by the Medical Clinic Administrator.

* Transport the appropriate documentation to each medical appointment. Return all documentation from the provider to The William George Agency Medical Department.

* Understand the medical issues that a child has and the reason for the appointment.

* Be able with good communication skills to be an advocate for the child during these off-campus appointments.

* Be an effective member of the treatment team demonstrating excellent communications and relationships between treatment team members, cottage director and manager, and the medical department.

* Be encouraged to engage in ongoing continuing education opportunities. Ex. First Aid and CPR Training.

* Assist medical office personnel in scheduling appointments in a timely and efficient manner.

* Maintain documentation in the medical record for each child at the William George Agency in accordance with established practices and NYS OCFS regulations.

* Assist nursing staff in maintaining inventory and organization of medical supplies.

* Assist nursing staff in maintaining a clean, safe and hygienic welcoming Residential Medical Clinic environment.

* Assist in scheduled functions at the Clinic when not engaged in transporting children. Ex. Helping with monthly pregnancy testing.

* Conduct monthly checks of on-campus AEDs. Change batteries as scheduled or as indicated.

* Conduct initial hearing and vision screening with appropriate documentation and recommendations for follow up appointments as indicated.

* Assist, transport and supervise as well as provide support for children during their appointments in the on-campus Mobile Dental Van.

* Make sure that all safety equipment in campus transport vehicles utilized by the Clinic are in working order and used appropriately during medical transport trips.

* Be prepared to work with independent healthcare providers on campus. Ex. Cayuga Medical Center Laboratory- blood draws. This also involves providing education and support for the population served at The William George Agency.

* Assist nursing and medical staff develop a comprehensive medical service plan for each resident.

* Attend and participate actively in Clinic staff meetings.

* Have a practical working knowledge of HIPPA Compliance and the in-house Health Information System.

* Ensure WGA Clinic Staff perform all duties and responsibilities in accordance with safety standards, practices and policies of the William George Agency.

* Perform other duties as assigned by the Medical Clinic Administrator and Nursing Supervisor.

* Provides back up to the Medical Office Assistant position.

POSITION LEVEL: Responsible to the Nursing Supervisor, for medical and Medical Clinic Administrator for administrative matters and scheduling issues.
